This is commonly overlooked by veterinarians and USDA officials, so it is the responsibility of the pet owner (or their pet relocation company!) to ensure that it is filled in. Thanks for your inquiry. You can find all of the import requirements and paperwork requirements for the Czech Republic on our website.
If you are only changing planes and not airlines (I'm guessing you're flying KLM?) then you will not have to fulfill any of the import requirements for Amsterdam as it will only be considered a transit.
